Besides the division winners, Baseball allows one other team (the remaining team with the best record, regardless of division) a playoff berth.

If they lose, and Cleveland wins, Boston and Cleveland have the same record, and will play a one game playoff (technically the first post-season game, I guess) to see who gets the right to go on as the fourth team in the playoffs.
Coltrane is out on Rush Street, so I'll take his proxy. He would appreciate if you were specific when you say "sox." at least for the next week until the White Sox get knocked out.
