I'm not disagreeing with your general proposition. However, there are countervailing health issues. In addition, there are also a significantnumber of studies that suggest that if someone is morbidly obese, no exercise or diet regimen alone will produce meaningful weight loss. For some people, surgical intervention will be their only real chance at weight loss.
This stuff isn't always as simple as you suggest.

i worked with a guy who was scared of cardiacs- family history of dead by 50 parents. he rode his bike to work, ate nothing unhealthy at all, and would run when he got home.
I saw him waiting for an elevator once and I knew he was going up 2 stories. I go "Andy, WTF? why wouldn't you walk up?" he told me that stairs are too intense and too short term so as to be more dangerous that helpful.
