 Sounds like you'll wear them to work with a tie on a daily basis.  Go with barrel cuffs; it won't look over the top for a day-to-day look, unless you can pull off Don Draper's style.  
Go to a Thomas Pink store to try on their shirts and figure out if fitted is a better cut for you than regular.  I really like the fabric for Pink shirts, but they're not inexpensive.  I find that Pink's fitted is far more trim than Brooks Brothers' fitted.  I got an e-mail from BB saying that they now have an even more fitted cut, though I'm not sure that is available in white dress shirts yet.  Charles Tyrwhitt's fitted is narrower than Pink or BB.  Watch for sales from CT.
Brooks Brothers shirts hold up well.  I haven't noticed CT shirts shrinking, but I usually wear them without a tie, so it wouldn't be a problem.  My advice might not be great on that point, as I actually would be fine if they shrank from laundering.
Banana Republic makes some really nice shirts with good fabric if you get their patterned Monogram line shirts.  Look carefully at their white dress shirts, as I've seen a range of fabrics and fit.  I'd stay away from buying them online, just so that you can see what you're getting.
