Not much has changed, except I made partner and moved firms. And I am filling out my application for naturalization to become a US citizen.

Re your question, I learned at a CLE course on immigration (taken just before my deadline a few years ago) that the questions on such forms haven't been updated in some time, so this one should be translated as "are you or have you ever been an alcoholic or addicted to illegal drugs?"
