I don't think I've had the MFM situation arise. I have had the FMF situation arise. In an admission I'm sure you'll find unsurprising, I screwed it up by saying something that offended one of the two. That mistake irks me to this day.
You're spot-on viewing this from a conquest perspective. Everybody I know who's done the MFM thing has joked about it later, or referenced it in some sort of male-bonding fashion. The stories resemble recollections of a concert or festival. (One truly epic one I heard started at a horse race.) It seems to be a "check the box" sort of thing for these people. But having seen only a couple friends' erections over the years, I can say unequivocally -- the vision of a friend's engorged member, even for that moment where you opened the door by accident while he and his girlfriend were going at it, is burned upon the brain.
Well, now I know what Bob's cock looks like hard... I wonder if I can wipe that image from the mind. Nope. No I can't. I don't see how anyone could have sex with a girl, with a friend, looking at that repeatedly, and knowing your friend was watching you.* And I say this as a person who has 0.000 modesty or hang-ups.

* This also applies to close female friends. I wouldn't want a really good female friend to watch me having sex, and I wouldn't want to watch her have sex. (Though if it were in a threesome with another woman, I'd think I'd bend that rule.)
