I think small firms have the greater issue with loose cannons. I've worked in 2 big firms, and they both had a central guy for conflicts issues, malpractice, etc. Everyone but the top rainmakers would go through them on any issues. Big guys sometimes got cocky and tried to sidestep, but the bulk of us plebes toed the line.
