There was a story in the legal press here in Philadelphia a few years back about a lateral partner who, IIRC, has a substantial book of business, being dinged by a BigLaw firm. The public* reason for the dinging was that his LS transcript wasn't up to snuff. Do forms really make prospective lateral partners produce a transcript? WTF?!?

* Of course, the actual reason could have been completely different. Either way, they hung this dude out to dry in a very public way.
