A good alternative -- a weekend away with her sister at a Spa before the baby arrives. It will be a long time before it happens again, and I understand that some spas have special stuff for pregos (and that there is other stuff pregos are supposed to avoid).
I was so freakin' huge with the Lexling that I would have hated anything that someone else bought if it didn't fit perfectly. And I hated anything that touched my stomach, (which was admittedly anything within about a 3' radius). I am tall and hard to fit for that reason anyway.

Assuming the BRSister is short, it may not be a big deal to buy her things. The weekend (or day) away is good, if she can offload the other child(ren) - so long as she stays out of hot tubs, of course. Or, offer to take in the BRNieces/BRNephews and send sis away for the day or send her and BIL out on a nice dinner/show date by themselves - a true luxury.
