Because it is more expensive to build interior walkways, and if you don't have effective interior security, the killer people could set bombs and/or break in and hide out and get you, and no one would be able to see to protect you.

You could have asked the question in such a way as to minimize the abortion aspect, like, "people who might get shot at on their way into work, like for instance ob/gyns who offer abortion services, seem like good candidates for having attached garages or something. And yet, on TV, it seems that many just have open parking lots. What gives?"
Oh, and perhaps they feel that it would be unfair to leave the patients all unprotected from gunfire when they themselves are protected.
