Obviously we don't know much that is
reliable about the facts, but it strikes me that it isn't that hard to use a condom (and I have a hard time understanding why you wouldn't want to do that for your own protection) and stop when you are asked.
That said, I also have a hard time understanding how a woman wouldn't/couldn't verify the presence of a condom before it, um, goes out of sight.
