Just used the estimation on the claim form.  But you were probably out of the country 5x what I was.  
 
The period was Feb 1, 1999 to Oct 15, 2004 (looked it up);  I think I said something around 30-35 days.  I believe that every single night of those was spent in a (non-budget) hotel charged to Amex.  So I got $15 (for having the card pre-99--per the settlement notice here , if you're interested) and all conversion fees charged (which I guess Amex actually documented).

I got $150, not $15.  
 
I didn't do anything more than (a) count the days I spent out of the country (maybe off by 1 or 2) in a five year period and (b) complete and sign a two page opt-out notice (which didn't require counting to 35, only chekcing a box) and, later, a two-page claim form.  
 
I've now spent more aggregate time posting here about it than I did completing the two forms.

Given that the form required name, address, card number, and a check box, why wouldn't  you spend five minutes to fill it out?  Even your billing rates aren't such a good return.
