The rules don't say this, but refs clearly take into account whether there was a chance at a goal. An unintentional handball on the goal line will be called. The same handball at midfield may not be.

Untrue. If it's unintentional but saves a goal, it will be. An example is Turkey's goal in the USMNT's warm-up game a couple of weeks ago. Geoff Cameron was standing on the goal line and the shot hit him in the arm. He wasn't trying to block it, but Turkey got the PK. Watch from the 6:40 mark here.
