Yes, on all counts. Back in the day, people used to use the term to describe those with Down's syndrome; I think the etymology is based on the quasi-epicanthic folds that some with Down's syndrome have (i.e., sort of slanty eyes). Obviously, not a term in current use (except by Sebby here, I guess).
