Depends on makers and wearers; fitted is better if you're getting custom tailored. Otherwise, the fact that they were cut to fit a manequin somewhere may or may not mean they fit you. Italians seem to have no chest and the French no wrists.
I'm sure Hank, at 6' 11", has the same problem.
Almost any off the rack shirt will pull at my wrists and shoulders; besides being uncomfortable, this means custom tailored shirts also last longer. And my tailor will even throw one or two in when he does a suit.
